FormIndex works fully offline. It does not request access to your camera, contacts, location, microphone, photos, files or any other sensitive device permissions.

5. Data Sharing

We do not share, sell, rent or transfer any user data to third parties, because no data leaves your device.

6. Data Retention and Deletion

Your data stays on your device until you delete it. You can:

Clear your weight log from Settings → Clear weight log

Remove all saved data from Settings → Reset all data

Uninstall the app, which permanently deletes all app data from your device

FormIndex provides general health and fitness information only. It is not a medical device and does not diagnose, treat, cure or prevent any disease or condition. Results are estimates based on standard formulas. Always consult a qualified doctor or healthcare professional before making decisions about your health, diet or exercise.

8. Children's Privacy

FormIndex is intended for adults aged 18 and over. The app is not directed at children under 13, and we do not knowingly collect any information from children.

9. Security

Because all information is stored locally on your device, its security depends on your device's own protections, such as a screen lock. We recommend keeping your device secure.
